  • A $50 no-show fee applies if you do not attend a confirmed imaging appointment. If you later reschedule through MARS, complete the rescheduled appointment, and the final imaging report is released, the $50 fee will be refunded.

    Mobile Imaging service location: The ZIP Code above is used as the service ZIP. Please enter the exact street address, city, and state where the mobile service is requested. The requested service radius must be at least 10 miles.

    Scheduling: Please allow at least 48 hours' advance notice for Mobile Imaging requests. Scheduling remains subject to available mobile service slots and final confirmation.

    Physician order / privacy: Do not upload, email, or enter physician orders, medical records, diagnoses, insurance identification numbers, Social Security numbers, or other sensitive health information through this standard website form. After submission, MARS will provide secure instructions for sending the physician order if it is needed.

    Employer verification: Employer-sponsored requests remain pending until MARS confirms eligibility with an authorized employer contact.
